The Arizona Department of Education requires selected students to have a Vision and Hearing Screening performed.
We have two certified and trained staff members within AzDL to assist with this screening.
It is required for parents/guardians to have their child/student attend one of these locations we have in place, IF the child/student is
- In the grade levels of either: Kindergarten, 1st Grade, 2nd Grade, 6th Grade, & 9th Grade (K,1,2,6,9)
- In Special Educationat any grade level
- New to Sequoia Choice Schools
- Struggling with vision or hearing issues and you'd like to request this be done even if they don't fall into the first 3 categories

Annual Notification to Parents Regarding Confidentiality of Student Education Records
Family Education Rights and Privacy Act (FERPA) is a Federal law that protects the privacy of student education records. FERPA gives parents certain rights with respect to their children's education records. These rights transfer to the student when he or she reaches the age of 18 or attends a school beyond the high school level. Students to whom the rights have transferred are "eligible students."
